Information and Requirements
Required Documentation Checklist for All New Registrants UPK-12
Identification & Residency:
- Parent/Caregiver Photo ID
- Student’s Birth Certificate
- Proof of Residency: Rental/Lease Agreement, Mortgage Statement, Utility Bill, etc.
Health & Medical:
- Student’s Immunization Records
- Copy of the child's most recent physical exam; specifically, “NYS Required School Health Examination”
*For students entering UPK and kindergarten only, the physical must indicate evidence of lead testing
Legal & Benefits (If Applicable):
- Custody Agreements or Court Orders
- New York State Benefits Card
