Documents needed at the time of registration:

· Proof of residency (tax bill, utility bill, or rental agreement)

· Custody agreement or court papers if applicable

· Original Birth Certificate

· Original Social Security card

· Parent or Guardian identification (usually, a driver’s license)

· Immunization record
